A mate of mine was having issues with cod4 muliplayer. Everytime he’d go to play the cod4 logo would load up then get the message of “iw3mp.exe has stopped working”
As always he found the solution on the net…

Haha, yeah I’ve seen that come up before… It is not the only cause of the problem mind you as many people play CoD4 multiplayer without a mic…
Well Post 4 is correct in a way as it was the Mic but it would of been his default settings in Windows. With Win trying to default to a broken mic, it wouldnt of picked up and thus when the game engine tried to Initialize the sound engine, it fell over. 
Its more a driver/settings issue than anything else
